VALUE-BASED health and social care

Value is measured by the improvement in the individual’s outcomes and experience of care relative to the cost of achieving that improvement.

Older people define the perceived value of care and the quality of care received, they are at the centre.

Team-oriented approach to care and data security is implemented, so care is coordinated, and patient outcomes can be measured easily.

Health care systems focus on improvement of staff satisfaction, achieving greater efficiency in the use of digital resources
