About the pilot

Target group:
In Ireland, The ValueCare Project seeks to co-design a digital health technology to support older people (≥75) experiencing mild or moderate levels of frailty in living well in their home setting, as well as their supportive network i.e. family caregivers and clinical team.

Stakeholders involved:
The pilot in Ireland is being conducted in Cork and Kerry region in Ireland and is being conducted in collaboration with the Ireland’s Health Service Executive (HSE) Integrated Care Programme for Older People (ICPOP). The pilot was appointed as one of the HSE’s Digital Health Living Labs in 2020. The ValueCare National Steering Group consists of representatives from ICPOP, the HSE’s Department of Digital Transformation and Open Innovation, Age Friendly Ireland and University College Cork.

Scope:
The digital health technology is a wellness plan supported by a virtual coach for older adults. This wellness plan is intended to promote health behavioural changes based on current guidelines in Ireland regarding physical activity, nutrition, medication adherence and social connectedness. Older adults will be recruited through the Integrated Care Hubs for Older People in Cork and Kerry. Care goals will be established with their multidisciplinary team before returning to their home setting. Participants will be receive educational content regarding their goals and provided motivational reminders to achieve their goals. Pilot partners

University College Dublin (UCD) leads the Cork-Kerry Pilot. UCD has a lot of experience managing multiple complex research projects which will be an advantage for the successful delivery of the project and the Irish pilot, which will be led by UCD. The University has strong links with the provider organisations with their health system and will leverage these links in the deployment of the pilot. UCD is developing a Centre for Healthcare Integration and its staff has significant experience in the design and implementation of Integrated Care.
